THE "MIRACLE ORTHOTIC"

One thing I’ve noticed over the years with people who have plantar fasciitis is a reduction in symptoms using arch supports in their shoes. This seems to work for years on some people, but eventually the pain returns and they think they need a new orthotic. But every new orthotic they try fails to give them any relief. They show me the orthotic that worked for years and it’s usually so stiff that it could survive a nuclear holocaust. Their orthotic didn’t wear out, but their foot has now developed a new, more intense, shortening along the plantar fascia of the foot that continued to worsen with the orthotic even though they didn’t have pain. These people often cannot walk at all without their shoes and orthotic. Granted these are some of the more severe cases. But the underlying, fundamental problem of their plantar fasciitis is never addressed.


WHAT'S THE UNDERLYING CAUSE

Imagine a rubber band with some elasticity in it. A normal, healthy plantar fascia should be able to stretch and retract with every step. When the plantar fascia becomes inflexible you can have plantar fasciitis. This inflexibility can be different along the length of the plantar fascia so that only parts of it hurt and not along the entire length from heal to toes. NEW BLOOD VESSEL GROWTH WITH LOW LEVEL LASER THERAPY

For many, the artery at the top of the foot needs to be lasered because it has branches that go down and feed the plantar fascia. Treating the top and bottom of the foot is important. The image to the right shows neovascularization (new blood vessels) delivery blood to plantar fascial tissue in a 64 year old female with plantar fascitis. She had tried orthotics, topical analgesic, oral anti-inflammatory meds, and stretching exercises with no results. Notice the extra red areas in the after image showing new blood vessel growth into the involved area. The before shows the lack of blood flow to the area which gives a “rigor mortis” type effect to the plantar fascia. Inflexible, painful to stretch, blood deprived, unhappy tissue. Low Level Laser Therapy restored circulation to the area bringing relief. What drug, what exercise, what stretch could do that? Answer is, it cannot. Even if shoe inserts take the pain away, it’s because they limit the stretch of the plantar fascia with each step. You need the blood supply restored to get to the root of the problem.


HOW LONG DOES IT TAKE TO NOTICE RESULTS?

It depends. If the stiffness in your plantar fascia is due to a lack of cellular energy from Cytochrome C Oxidase problems alone or is it also due to a lack of blood vessels supplying blood to the plantar fascia. If it’s only the first one, then symptoms change quickly. If it’s both, then symptoms change slowly because it takes time for new blood vessels to grow. LLLT corrects the cellular energy problem that allows both problems to get handled. But if you’re three or four treatments into LLLT without noticing change don’t be discouraged. As you can see from the study below it took time to get results months after the last LLLT treatment. This video explains why it can take time after you’ve finished your treatments.

LOW-LEVEL LASER THERAPY FOR THE TREATMENT OF CHRONIC PLANTAR FASCIITIS

BACKGROUND:
Plantar fasciitis affects nearly 1 million people annually in the United States. Traditional nonoperative management is successful in about 90% of patients, usually within 10 months. Chronic plantar fasciitis develops in about 10% of patients and is a difficult clinical problem to treat. A newly emerging technology, low-level laser therapy (LLLT), has demonstrated promising results for the treatment of acute and chronic pain.

RESEARCH STUDY METHODS:

Thirty patients were administered LLLT and completed 12 months of follow-up. Patients were treated twice a week for 3 weeks for a total of 6 treatments and were evaluated at baseline, 2 weeks post procedure, and 6 and 12 months post procedure. Patients completed the Visual Analog Scale (VAS) and Foot Function Index (FFI) at study follow-up periods.

RESULTS:

Patients demonstrated a mean improvement in heel pain Visual Analog Scale from 67.8 out of 100 at baseline to 6.9 out of 100 at the 12-month follow-up period. Total Foot Function Index score improved from a mean of 106.2 at baseline to 32.3 at 12 months post procedure.

CONCLUSION:

Although further studies are warranted, this study shows that LLLT is a promising treatment of chronic plantar fasciitis.
